Boomerang Shaped Neck Cyst: Is it a thyroid cyst? How I treated it with ethanol PEI.

34 Y/O male with choking sensation. Ultrasound and soft tissue images revealed a boomerang shaped cystic mass encircling the right lobe and isthmus area. Consultations recommendations  with surgeons were for surgery. No one mentioned ethanol ablation and one physician said PEI was dangerous to the recurrent nerve and painful.  FNA obtained brown cyst fluid  for Thyroglobulin washout. The TG was 16,791 confirming thyroid origin of the cyst.The cyst was 80 ml in volume. After drainage and ethanol ablation that reduced to 3.8 ml. A 95 % reduction in cyst size. The symptoms were resolved. The unusual shape of the original cyst is now a slit like small cyst.

Depending on how much cyst fluid I either treated with ethanol ablation if mostly cystic and radiofrequency ablation if > 50% solid.
