Thyroid (1) 2022 Tuttle RM et al
Dr.Guttler’s comments:
-
Study looked at 943 lobectomy cases matched with 943 patients with a total thyroidectomy for outcomes.
-
At ten years there was no difference what surgery they had.
-
Previous article by Tufano in the same Thyroid Journal found thyroid radiofrequency RFA was as good as surgery for micropapillary cancers, with less pain, no scar, loss of work, less expensive, and social and emotional outcomes were better.
-
What does that say about the state of thyroid surgery in 2020?
-
Less surgery is more. Lobe is as good as a total, and even better no surgery for selected small papillary thyroid cancers for thyroid RFA..
-
If you need surgery less is more. A lobectomy is just as good as a total thyroidectomy < 4 cm low/medium papillary cancers.
-
You don’t need surgery for selected small <1.5 cm papillary cancers as Dr Tufano’s paper in Thyroid showed data that radiofrequency ablation is a good as surgery with less negative effects.
