Large Benign Thyroid Cyst Treated by Ethanol Ablation PEI.

The patient refused surgery for this  very large thyroid cyst.b973881d-77b3-449e-bab1-8ec3ff07f8e4-cyst

64 ml cyst had 30 cc Cyst fluid removed and a cutting needle biopsy was done of the cyst capsule.

It was a benign colloid cystic nodule. The unusual location suggested a possible parathyroid cyst but washout for PTH was negative while TG was 16,000 making thyroid cyst the diagnosis.

 

 

Under US guidance the 18 gauge needle was inserted into the center of the cyst.

total of 35 ml cyst fluid removed after saline washout to loosen the thickened residual  fluid.  This was followed by injection of 18 ml 200 proof ethanol.After 10 minutes the ethanol was removed. The patient was observed for 45 minutes after without complications or pain. 3 days later a call to him in good spirits without symptoms or pain.

PEI is the treatment of choice for thyroid and parathyroid cysts. Surgery is not the preferred treatment method. Do not let your physicians and surgeons tell you surgery is the only choice as PEI is painful and dangerous. PEI has almost no major complications and 1-3% minor ones.
