Hypothyroidism: When Your Thyroid Is Underactive

Hypothyroidism occurs when the thyroid gland does not produce enough thyroid hormone to meet the body's needs. Causes of Hypothyroidism

Symptoms

Common symptoms include fatigue and sluggishness, increased sensitivity to cold, constipation, pale and dry skin, brittle nails, hair thinning or loss, slowed heart rate, depression, impaired memory and concentration, weight gain despite no change in diet, muscle weakness and aches, and elevated blood cholesterol. In women, heavy or irregular menstrual periods are common. Symptoms often develop gradually over years.

Diagnosis: The Role of TSH

The primary screening test for hypothyroidism is the TSH (thyroid-stimulating hormone) blood test. An elevated TSH indicates the pituitary is working harder to stimulate an underperforming thyroid. Free T4 levels confirm the diagnosis. Dr. Guttler emphasizes that the optimal TSH range for treated patients — particularly those on T4 therapy — is a nuanced clinical judgment, not simply a number within the laboratory reference range.

Treatment: Levothyroxine and Beyond

The standard treatment is daily levothyroxine (synthetic T4), a safe and effective medication when properly dosed. The goal is to normalize TSH and relieve symptoms. Some patients feel better with combination T4/T3 therapy (adding liothyronine or using desiccated thyroid extract such as Armour Thyroid).
